EMERGENCY BENEFIT ASSISTANCE PROGRAM
- Ambulance Services Php 1,000.00
- Hospitalization Assistance PhP 1,200.00/day
• Minimum Period of Confinement: 36 hours or 1½ day
• Maximum Period of Confinement: 720 hours or 30 days
• Maximum Assistance per Confinement: 5 days only
• To be availed ONLY after 3 months of Affiliation; First 12 Hours of Confinement – NOT COVERED
• Executive Check-up is NOT covered by the Benefit Assistance.
• Assistance is based on the published List and Classification of Illnesses and Diseases.
- Supplementary Medical Assistance I PhP 7,500.00 (Lifetime Limit)
For Major Surgery with General Anesthesia i.e. Appendicitis, Myoma, Gastro Intestinal Lymphoma, Hemorrhoids, etc. To be availed after Six (6) months of Affiliation. In excess of Philhealth and HMOs.
- Supplementary Medical Assistance II PhP50,000.00 (Lifetime Limit)
For Critical Illness and Dreaded Diseases limited to Open Heart Surgery, Kidney/Liver Transplant, Cancer cases with Surgery and/or Chemotherapy. To be availed after Nine (9) months of Affiliation. In excess of Philhealth and HMOs.
• Both Supplementary Medical Assistance will require official ORIGINAL receipts, statement of account and medical records; Inclusive of Daily Hospitalization Assistance.
- Additional Burial Assistance PhP10,000.00 (Maximum)
To be availed after one (1) year of membership
• All Medical Reimbursement in excess of Philhealth and others plans will require ORIGINAL RECEIPTS.
• Burial Assistance will require original Death Certificate.
• Exclusions: Injuries and Deaths Due to War, Civil Strife, Rebellion, Coup d’ etat, Murder and Assualt, Acts of Terrorism, Illegal and Self-inflicted Acts, Maternity Related, Chronic and Pre-existing Conditions.
• All applications for Benefit Assistance are subject to review of the Benefit Assistance Committee.
• SAGIP KA 2000 Foundation reserves the right to make adjustments in annual contributions. Benefit Assistance is based on the approved guidelines for a particular period the member is affiliated.
