FOR STUDENT VOLUNTEERS PERSONAL ACCIDENT
CONTRIBUTION: Php 0.25/day or Php 90.00 per year
AGE LIMIT: 5 – 65 Years Old
EMERGENCY BENEFIT ASSISTANCE
(Exclusive for Sagip Ka 2000 Volunteers)
Coverage/Max. Limits
- Medical Reimbursement due to Accident PhP 5,000.00
- Bereavement Assistance (Natural Death) PhP 7,000.00
- Accidental Death PhP50,000.00
Terms & Conditions:
1. 100% of Student Population enrolled
2. Master List of Students with date of birth and age
3. Free coverage ONLY for full-time administrator, faculty and staff
4. 24-hours; 365 days coverage
5. Official original receipts and Medical Certificate required for medical reimbursement purposes.
6. Police Report, Medico-Legal Report, Death Certificate, etc. are require in case of accidental death.
7. Certified true copy of the Death Certificate will be required for bereavement assistance.
